There is a particular kind of exhaustion many women begin to recognise in their forties.
You feel deeply tired, yet your body refuses to switch off. You fall asleep but wake at 3am. Your mind is quiet but alert enough to prevent deep rest. By morning you feel as though the night never restored you.
Many women assume this is simply stress or ageing. In reality it is often a rhythm issue involving the HPA axis, the communication loop between the hypothalamus, pituitary and adrenal glands that regulates cortisol.
What the HPA Axis Actually Does
At the centre of this experience is the HPA axis, the communication loop between the hypothalamus, pituitary gland and adrenal glands. This system regulates cortisol, your primary stress hormone.
Cortisol is not the enemy. It is a rhythm hormone.
In a well-regulated system, cortisol rises in the morning to wake you and mobilise energy. It gradually declines throughout the day and falls in the evening to allow melatonin to rise and sleep to begin.
Cortisol and melatonin function almost like a seesaw. When one rises, the other falls.
Research in chronobiology shows that cortisol secretion follows a tightly regulated circadian rhythm, with a natural peak shortly after waking and a gradual decline toward night, allowing melatonin to initiate sleep (Buckley & Schatzberg, 2005).
When this rhythm becomes unstable, the nervous system begins to feel constantly alert.
Stress Hormones Rarely Act Alone
When women experience the tired but wired pattern, cortisol is rarely acting alone.
In clinical practice I often see this pattern emerging from the interaction between stress hormones, blood sugar regulation, sleep rhythms, immune mediators such as histamine, and thyroid signalling.
Why Perimenopause Changes the Stress Response
Perimenopause introduces a new variable into this system.
Fluctuating oestrogen changes how the brain perceives and responds to stress signals. At the same time, progesterone, which normally supports calming neurotransmitters such as GABA, begins to decline.
The nervous system therefore becomes less buffered.
What you could override at 35 becomes much harder to override at 45.
Research suggests that oestrogen modulates the brain’s response to stress hormones and emotional regulation, which helps explain why stress sensitivity often increases during the menopausal transition (Gordon et al., 2015).
The result is not weakness.
It is increased physiological sensitivity.
The Two Cortisol Patterns We See in Midlife
One of the most important things to understand is that not every woman experiencing fatigue has the same cortisol pattern.
In clinical practice we often see two distinct physiological presentations.
Understanding the difference is essential, because the interventions are very different.
High Cortisol Pattern: The Overstimulated System
In this pattern the stress system remains overly activated.
Women often describe:
- difficulty falling asleep
- waking during the night
- feeling anxious or overstimulated
- sensitivity to caffeine
- palpitations or adrenaline-like hot flushes
Even though you feel exhausted, the nervous system remains on alert.
Evening cortisol may remain elevated, suppressing melatonin and fragmenting sleep.
This is the classic “tired but wired” state.
What this system needs is not stimulation.
It needs containment and rhythm.
When cortisol fails to fall properly in the evening, melatonin cannot rise. This is one of the most common causes of the 3am waking pattern I see in clinic.

Low Cortisol Pattern: The Flattened System
For other women the experience looks very different.
Instead of feeling wired, they feel flat.
Morning energy is low. Getting started feels effortful. Motivation dips and afternoon crashes become common.
In this pattern the stress system has often been pushed for years.
The morning cortisol rise becomes weaker and the body struggles to mobilise energy efficiently.
Instead of calming the system further, the goal here is gentle rebuilding.
When the stress system remains activated for years, thyroid signalling can also shift.
This is one reason fatigue is sometimes misattributed purely to hormones.

Blood Sugar and the Stress System
Cortisol plays a critical role in maintaining stable glucose levels.
When blood sugar drops, cortisol rises to compensate.
This means that skipping meals, relying on caffeine, restricting carbohydrates aggressively or exercising intensely while under-fuelled can repeatedly activate the stress response.
The body does not distinguish between emotional stress and metabolic stress. Blood sugar crashes activate the same cortisol response as psychological stress.

Both activate the HPA axis.
This relationship reflects the body’s survival system. When glucose levels fall, cortisol stimulates glucose production to maintain brain fuel, linking metabolic stress directly with activation of the HPA axis (Joseph & Golden, 2017).
This is why stabilising blood sugar is not simply a dietary strategy.
It is a nervous system strategy.
Histamine and the Stress Loop
Another layer often appears in women who feel overstimulated.
The immune system and nervous system constantly communicate.
Elevated cortisol can stimulate mast cell activity. Histamine release can in turn stimulate cortisol production.
This creates a loop where anxiety, flushing, digestive reactivity and sleep disturbance reinforce one another.
Many women interpret this as losing resilience.
In reality, it is a neuro-immune feedback loop.
Many women who feel “tired but wired” also notice flushing, anxiety, headaches or digestive reactivity. These symptoms are often connected to histamine signalling and mast cell activation.

Rebuilding Rhythm
The goal in my work is never to suppress cortisol.
The goal is to restore rhythm.
This usually begins with simple physiological anchors:
- regular meal timing
- adequate protein intake
- morning light exposure
- strength-based movement rather than excessive cardio
- consistent sleep routines
When rhythm returns, the nervous system often softens naturally.
The body was never broken.
It simply lost its timing.
