Many women arrive in midlife with a quiet confusion. 

Their periods are heavier.
Their breasts feel tender.
Their moods swing more sharply.
Their body feels puffier, more reactive, less forgiving. 

And often, they are told the same thing: 

“Your oestrogen is high.”
“Your hormones are normal.”
“Welcome to perimenopause.” 

But what if oestrogen dominance in midlife is not really about having too much oestrogen at all? 

What if it is about how your body clears, processes, and responds to it?

Oestrogen Dominance Is Rarely About Excess Production



In perimenopause, oestrogen does not simply decline in a straight line.
It fluctuates. Sometimes dramatically. 

Progesterone, however, tends to fall earlier and more consistently. 

This creates a state of relative oestrogen dominance, where oestrogen’s effects feel stronger because progesterone is no longer buffering them. 

Yet many women are surprised to learn that their blood tests can still appear “normal”. 

This is because standard testing often looks at isolated hormone levels, without considering metabolism, clearance, receptor sensitivity, or timing. The question functional medicine asks is not “How high is your oestrogen?”
It is “What is happening to oestrogen once your body has used it?” 

The Liver Is Not the Villain. It Is the Bottleneck.

Your liver plays a central role in hormone clearance.

Every day, it packages used oestrogen so it can be safely eliminated through the gut. But in midlife, this system often slows, not because the liver is weak, but because it is overloaded. 

Environmental toxins, alcohol, medications, chronic stress, poor sleep, blood sugar swings, and nutrient depletion all compete for the same detoxification pathways.

When clearance slows, oestrogen is not removed efficiently. It lingers. It recirculates. And its effects amplify.

The Gut. Where Oestrogen Can Either Exit or Return.

Once the liver has done its job, oestrogen still has one more hurdle: the gut. 

The oestrobolome, a collection of gut bacteria, helps regulate whether oestrogen is excreted or reabsorbed. If gut function is compromised through constipation, dysbiosis, inflammation, or sluggish motility, oestrogen is more likely to be pulled back into circulation. 

This is one reason why women with bloating, irregular stools, or digestive discomfort often experience more intense hormonal symptoms.

The Nervous System. The Silent Amplifier.

There is another layer often overlooked. 

Your nervous system. 

Chronic stress does not just raise cortisol. It changes how your body responds to hormones. 

When the nervous system is in a state of ongoing alert, tissues become more sensitive. Reactions intensify. Symptoms feel louder. 

This is why many women notice that anxiety, irritability, or emotional volatility increase alongside hormonal symptoms. Often, it is not that hormones are “worse”, but that the system interpreting them is under strain.

Holding on.

One of the least discussed contributors to oestrogen dominance is energy depletion. 

Detoxification, gut motility, nervous system regulation, and hormone metabolism all require energy. When a woman is running on empty, her ability to clear hormones efficiently diminishes.
