It did not happen overnight.

For me, it began with wine.

I used to tolerate red wine perfectly well. Then one evening I noticed I felt flushed and foggy after just one glass. My heart felt slightly faster. My sleep was restless. So I switched to white wine. That seemed better for a while. Then even that started to trigger headaches and that familiar wired feeling behind my eyes.

Eventually, I stopped drinking wine altogether.

At the time, I did not think histamine.
I thought stress. Ageing. Coincidence.

Years later, in clinic, I began hearing the same story from women in perimenopause.

“I cannot tolerate foods I used to eat.”
“My anxiety feels sharper.”
“I flush for no reason.”
“My heart races randomly.”
“I feel reactive… to everything.”

And that word is important.

Reactive.

Because perimenopause is not only a hormonal transition.
It is often an immune transition.

Histamine intolerance in perimenopause can present as flushing, itching, bloating, headaches, palpitations, anxiety, sinus congestion or sleep disruption, often worsening around hormonal shifts.

And histamine sits at the centre of that shift.

When Sensitive Is Not Psychological

Histamine is not an enemy. It is a natural immune messenger.

It helps regulate inflammation, stomach acid, blood vessel dilation and wakefulness. It is released by specialised immune cells called mast cells, which sit in the skin, gut lining, respiratory tract and near nerve endings.

Under balanced conditions, histamine is released when needed and then efficiently cleared.

But perimenopause changes the terrain.

Oestrogen begins to fluctuate more dramatically. Progesterone declines. Stress tolerance often decreases. Sleep becomes lighter. Blood sugar regulation may become less stable. Gut balance may shift.

And here is the key mechanism.

Oestrogen stimulates histamine release.
Histamine can stimulate further oestrogen production.

This creates a feedback loop.

When progesterone declines, the calming modulation of mast cells weakens. The loop becomes louder.

What once felt manageable now feels amplified.

Not because you are fragile.
Because the signalling is stronger.

The Oestrogen-Histamine Loop in Real Life

This mechanism is not abstract. It shows up in patterns.

Anxiety that peaks around ovulation.
Flushing before a period.
Migraines tracking hormonal shifts.
Sudden intolerance to wine or fermented foods.
Sleep that feels wired rather than restful.

Histamine is inflammatory, but it is also stimulating. It activates parts of the nervous system responsible for alertness. When levels rise in a hormonally sensitive body, the result can feel like:

Wired but tired.
On edge.
Internally inflamed.

In clinic, I see this pattern most often in women between 42 and 50 who describe themselves as “suddenly sensitive” despite having previously felt resilient.

Many women describe perimenopause as “my body feels louder.”

Often, it is.

When Midlife Makes Reactivity More Likely

Perimenopause rarely acts alone. It interacts with the terrain you have built over decades.

If blood sugar swings are frequent, inflammatory signalling increases. Insulin spikes do not just affect weight; they influence immune reactivity. As discussed in The Hidden Link Between Hormones and Blood Sugar, metabolic instability amplifies inflammatory pathways.

If stress has been chronic, mast cells remain on alert. Cortisol fluctuations, explored in The Cortisol–Oestrogen Dance, alter immune balance and reduce stress resilience.

Add fluctuating oestrogen and declining progesterone to this environment and the system becomes primed.

Not broken.

Primed.

When Does This Become Mast Cell Activation Syndrome?

Not every woman with histamine sensitivity has Mast Cell Activation Syndrome. MCAS is a more complex condition where mast cells release mediators excessively and systemically.

However, perimenopause can unmask underlying mast cell vulnerability.

If symptoms affect multiple systems, skin, digestion, cardiovascular, neurological and feel disproportionate or unpredictable, deeper investigation may be warranted.

This is where personalised assessment matters.

Not guessing.
Not over-restricting.
Not dismissing.

Understanding the pattern.

The Common Mistake: Restricting Everything

When symptoms feel inflammatory, many women respond by eliminating aggressively.

No wine.
No aged foods.
No leftovers.
No tomatoes.
No fermented foods.

Soon the list becomes exhausting.

But histamine intolerance in menopause is rarely just about histamine in food.

It is about total load.

Load from hormones.
Load from stress.
Load from gut imbalance.
Load from sleep disruption.
Load from inflammation.

Restriction alone does not lower systemic load. It often increases stress, which can worsen mast cell reactivity.

Calming the Terrain

Histamine reactivity softens when the terrain stabilises.

That means regular meals that steady blood sugar.
Adequate protein to support detox enzymes.
Gut repair to strengthen DAO activity.
Nervous system regulation to reduce stress signalling.
Sleep that allows inflammatory pathways to settle.

This is slower than elimination. But it is more sustainable.

And it respects the intelligence of your physiology.

A Personal Reflection

Looking back at my wine intolerance, I see it differently now.

It was not my body turning against me.
It was my system signalling that overall load had exceeded capacity.

When I addressed stress, gut balance and hormonal rhythm together, tolerance improved. Not perfectly, but predictably.

That pattern has repeated itself in many women I support.

Reactivity decreases when total burden decreases.
